If you require a prescription and think the doctor will supply it without seeing you,please submit a written request, fax (01344 301862) or e-mail with the name of the drug and if possible the condition requiring treatment.

Please allow 2 full working days for processing the prescription before collection. You may be asked to attend the surgery at intervals to monitor certain drugs or conditions.
