Easthampstead Surgery - Information for new patients wishing to join the doctors surgery

New Patient Registration

stethescope The doctors welcome new patients who live within the Bracknell area.

As it often takes sometime for full paper records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire.

We participate in the GP2GP service so if your previous Doctor is also on GP2GP your electronic notes should be recieved with 48 hours and then you will be fully registered to see a Doctor.

For registration we will need photo ID and proof of address. Your NHS number and previous Doctors details will speed up the registration process.
